Transition in Paediatric Liver Transplant Recipients

Transition of care is a critical process for paediatric liver transplant recipients as they move from child-centered to adult-oriented healthcare services. This period is not merely a transfer of medical responsibility, but a structured, multidisciplinary approach aimed at fostering autonomy, adherence to treatment, and long-term graft survival. The group in mainly focused in ensuring a structured transition progress for adolescent liver transplant recipients able to measure effectiveness of transition, to reduce dispersion and non-adherence. Moreover, the group is focusing in evaluating outcomes after transition, in terms of long-term allograft health and patients’ global outcomes.
